A 17-year old Tallahassee, Florida girl was arrested and charged with manslaughter after she and a group of kids were passing  around a gun in a local apartment.  It apparently went off while she was holding it and a 17-year old boy was killed.  The news report does not say how the teenagers got the gun or whether they were aware that the gun was loaded.  Nor does it state whether the gun was equipped with a loaded chamber indicator.

The Tallahassee Democrat reports that this was the 15th death in at least 68 shootings in Tallahassee this year.  This epidemic of shootings “and a surge of teens bringing firearms onto school grounds” prompted a new “Stop the Violence” campaign in Leon County Schools.
