Michael McDougal of Columbus, Georgia, age 26, was killed in a gun accident that reportedly occurred when his 9-year old daughter placed a gun on a table.  There is an ongoing investigation. 

The report is unclear about whether the trigger was pulled. There are several well-known defects that cause certain makes and models of guns to fire without a trigger pull.
